Central Health Partnership PCN was formed to provide better healthcare for patients and better support for GP practices in the Sandwell area. As a collective, we can focus on providing exceptional GP services for patients while managing our practices innovatively, efficiently and effectively. We Bring together 7 GP practice to work as a team to provide better care for our communities.
